Thank you for attending WA ACDA SI 2021 with headline guest, Dr. Julie Yu. Please help us improve our offerings at WA ACDA Summer Institute by taking the time to fill out our participant survey.
Survey & Clock Hour Registration Form
The form also includes Clock Hour Registration. We are an approved Clock Hours provider with OSPI, and are glad to offer 19 maximum clock hours to our members for no charge, if you attend the whole conference. You may not apply for hours you did not attend. Adjust your hours according to your attendance. You may choose to fill out the form on Friday at the completion of our conference, or daily at the completion of each day. This form will open at 5pm on Wednesday, July 21. On the confirmation page of your submission, you will be given a LINK to the PDF of your clock hours form.
